Fire Permit Forms and Applications

ADHS Licensed Facility Permits, Educational Facility Permits, and Blasing/Pyrotechnian Certified of Fitness Cards

Fire Prevention has transitioned to a new time savings and customer-friendly permitting system called Shape PHX , effective June 6, 2022, for state-licensed​ operational permits, Educational Facility Permits, and Blasting/Pyrotechic Cardholder Certificates.​ This system provides an enhanced user experience with easier navigation and more self-service options for you, our customer. Shape Phx manages application intake, inspection requests, workflow, permit status, and reports. This new secure system significantly benefits our customers through improved customer service, added transparency, and increased efficiency of services that the Phoenix Fire Department provides to our community. The best part of Shape Phx for our customers is the “Shape Phx Customer Portal." 

The Portal is available 24/7 and allows customers that register to do the following from any location; submit applications, check plan review status, request meetings with staff, upload documents, request inspections and pay fees online. 

If your application request is for one or more of the below permits, you will now need to complete the application process online. 

Applications for these permits will no longer be accepted via any other method for processing. 

Instr​uctions to apply for a new Fire permit/inspection in Shape PHX

​Use this if you have never had a permit issued from Shape PHX even if you have an existing business.

Instructions to renew a Fire permit/inspection in Shape PHX permit​ ​

​​Use this if you are renewing a Shape PHX permit. Shape permit numbers do not change, a renewal will use the existing AOP (Annual Operating Permit) number and add the year at the end.

Construction Plan Review and Permit Forms

​​​​ When applying for a permit with Fire Prevention it is important to note the following are required as part of the submittal and permit application process.

  • ​Site Plans & Fire Protection Systems require hard copy plan submittal. Documents shall be drawn to scale and printed to scale.​
  • Electronic media documents are allowed to be submitted where approved by the fire code official for any non-life safety equipment. 
  • Construction documents shall be of sufficient clarity to indicate the location, nature and extent of the work proposed and show in detail that it will conform to the provisions of this code and relevant laws, ordinances, rules and regulations.
  • Two sets of accurate and legible plans shall be submitted to the fire code official for approval prior to the issuance of a permit, in the following scale:  Site Plans [1 inch = 50 feet]       Fire Protection Systems [1/8 inch = 1 foot]  Use for: Hydrant Flow Test, Fire Sprinkler, Fire Pump, Standpipe, Alt. Surface Fire Apparatus Access, Fire Alarm, Fire Line/ Civil, Kitchen Hood
  • Other scales may be used with prior approval from the fire code official.
  • Submittals require a bar graph scale on each page and a directional arrow.
