The Phoenix Police Department has released a Critical Incident Briefing (CIB) video that includes audio, visuals and information related to an officer-involved shooting (OIS) which occurred on October 11, 2025.

The incident began around 8 p.m. when patrol officers responded to a report of a fight at an apartment complex located near 83rd Avenue and Indian School Road. Multiple calls to 911 came in describing a man armed with a machete assaulting his wife and threatening residents who were trying to call 911.

One officer arrived and saw a man armed with a machete holding a woman on the ground. The officer ordered the man to release the woman. The man continued to hold the machete to the woman’s neck. The officer then fired his handgun, striking the man. After the shooting, the officer was able to pull the woman to safety.

The fire department responded and determined the man died from his injuries on scene. The woman suffered minor injuries and was treated and released on scene. No one else was injured during this incident.

The machete was recovered on scene by investigators.

The officer involved in the shooting is assigned to the Maryvale Estrella Mountain Precinct and has six years of law enforcement experience, with less than a year with Phoenix PD.

This incident is the subject of a criminal investigation being conducted by the Department of Public Safety’s Major Incident Division.

The investigation will be reviewed by the Maricopa County Attorney’s Office. It is also the subject of an administrative investigation being conducted by the Phoenix Police Department.

Conclusions about whether the actions of the officer are consistent with department policy and the law will not be made until all facts are known and the investigation is complete.
