The purpose of this checklist is to offer comments on plan design for construction of traffic signals within public right-of-way. Source of traffic signal and related construction in the public right-of-way design policy are the latest version of City of Phoenix Specifications and Details, MAG, AASHTO, MUTCD, and City of Phoenix Traffic Signal standards (Design Procedure Manual)
This checklist serves to minimize redline comments on the check prints and to maintain consistency of the plan review process for traffic signal and related construction in the public right-of-way. Plan approval, and issuing permits depend on the compliance with the comments made on the check prints and this checklist. The engineer of record shall satisfy themselves of the completeness and accuracy of the design.
Please return this checklist and the check prints with your next submittal. Discussion of redlines comments on plans or this checklist should be directed to the plan reviewer listed above.
Plan approvals are valid for a period of one (1) year from the date of the approval signature or as otherwise specified by the Street Transportation Signal Section
The following symbols are used to identify changes needed to the plans.

GENERAL REQUIREMENTS:
Setup a field review meeting with the Signal Shop prior to start of design at (602) 262-6733
Obtain most current standard details and AutoCAD standards from The Street Transportation Department (602) 256-3409
Include an AutoCAD 2002 electronic drawing with every submittal
Set-up a pre-job meeting prior to start of construction at (602) 262-6733
Please invite the following: Contractor, Development Services Inspector, Traffic Signal Inspector, Traffic Signal Supervisor and City of Phoenix Utility Inspector.
Project title block filled out with name and location of project.
Appropriate processing numbers including:
KIVA#, COP File #, in lower right corner.
Submit 2 sets of signal plans sheet size 24” X 36”; These sets will include an underground foundation (sheet 1) and an aboveground equipment (sheet 2).
Submit 6 sets of signal plans sheet size 11” X 17”; These sets will include an underground foundation (sheet 1) and an aboveground equipment (sheet 2).
Block symbols shall be per current City of Phoenix AutoCAD standards specifications and Design Procedure Manual.
Provide plans on reproducible media for submittal.
All sheets must have the Civil Engineer’s Arizona registration seal and original signature prior to plan submittal.
City of Phoenix Street Transportation Department layer standards must be maintained per the current AutoCAD standards
City of Phoenix Street Transportation Department line types must be used per the current AutoCAD standards.
City of Phoenix Street Transportation Department Traffic Signal Template must be used (File “signal.dwt”)
Show dimensions for FOC on all intersection street legs per the current City of Phoenix standards.
Show dimensions for ROW on all intersection legs per the current City of Phoenix standards.
Show dimensions for required new traffic control easements per the current City of Phoenix standards.
Blue stake stamp shown on all plans.
Provide a copy of ctb plot file if different than the one provided by the City of Phoenix
Add this note to all plans “The Contractor shall call the City of Phoenix Signal Shop a minimum of 2 working days to request blue staking of existing facilities.
The Contractor shall call for blue stake for all other existing facilities.”

Locate the intersection drawing in the NAD 83 coordinate system
In order to accomplish this item, you will need to obtain a copy of the appropriate quarter section(s)
Contact the City of Phoenix Street Transportation Department for copy of existing signal plans if available, otherwise the intersection will need to be redrawn
Engineer’s name, address, and telephone number
Power run must be installed per APS or SRP standard specifications. Existing power location provided by the Street Transportation Department
Provide a Telco location.
Obtain a proposed power address from the Street Transportation Department (602) 256-3409
Show City Of Phoenix ordinance G-4396 on all sheets.
"Per City of Phoenix Ordinance G-4396 , these plans are for official use only and may not be shared with others
except as required to fulfill the obligations of your contract with the City of Phoenix ”
Maintenance of traffic plan must be submitted per City of Phoenix Standards.
All signal drawing plots will be plotted at 1 = 20
SHEET 1
FOUNDATION SHEET CHECKLIST:
Should City require right-of-way for tapers, construction easements, traffic control easements, temporary turn-arounds, and /or___________, right-of-way documents are required. If this right-of-way is not included on plat or map of dedication, it will need to be dedicated by a separate instrument. Call (602) 262-6733 for information on separate dedications. These items should be submitted as soon as possible. A minimum of six to eight weeks is required for processing and no right-of-way permits will be issued until dedication is complete
Show all existing traffic signal conduit and junction boxes
Do not Show existing conduit and junction boxes that will be removed
Show the location of proposed traffic signal conduit and junction boxes. Traffic signal improvements will be reviewed and approved by the Street Transportation Department/Planning, Design, & Programming Division
Show all existing underground utilities

If a conduit plan is being created, the title block shall read:

If a conduit plan is being created, run conduit to monument lines or match existing conduit. All splices must be approved by the Street Transportation Signal Section
If a modification signal plan or a new signal plan is being created, the title block shall read:
(line 1) TRAFFIC SIGNAL FOUNDATION, (line 2) INTERSECTION DESCRIPTION
Foundation location shall be determined per Design Procedure Manual or as specified by the Street Transportation Traffic Signal Engineer

1 No. 5 junction boxes must be installed 5’ from median island nose
1 No. 5 junction boxes must be installed on three corners
1 No. 7 junction box must be installed on corner where proposed power is located
All main junction boxes shall be connected by 2- 2 ½” schedule 40 gray PVC
Main junction boxes to foundations shall be connected by 1- 2 ½” schedule 40 gray PVC (Unless Ootherwise Specified)
Main junction box to 3 ½ loop detector junction box shall be connected by 1- 2 ” schedule 40 gray PVC
No. 7 junction box to power source shall be connected by 1- 2 ½” schedule 80 red PVC
Provide a power service pedestal (PSP) within 5’ of the power source unless otherwise specified by the Signal Shop
Number pole foundations sequentially, within a hexagon symbol (block "STB048"), starting with the foundations and rotating around the intersection
in a clockwise direction from the Northwest corner
Number pull junction boxes sequentially, within a hexagon symbol (block "STB048"), continuing with the same number sequence used for the pole foundations in a clockwise direction around the intersection
In the “INSTALL” legend, foundations and junction boxes must be referenced to the matrix
List the length of conduit in linear feet for each size of new conduit used in the intersection. Existing conduit will be listed in the install note but does not need a quantity
Foundations to be removed shall be numbered within a square symbol (STB049) and listed under the “REMOVE” legend

EQUIPMENT SHEET CHECKLIST:
Show dimensions for all new and existing loop detectors per the Street Transportation Department Design Procedure Manual
Striping at the intersection must match the overall project striping.
Existing equipment that will be removed by this project is not to be shown in the plans
If a modification signal plan or a new signal plan is being created, the title block shall read
